Attention Parents/Guardians of Current Kindergarten Students:
All children entering first grade in the fall need to have a complete physical exam. The examination is required by California State Law. The examination helps to uncover any hidden health problems which might keep your child from doing well in school.
Children may have the exam as early as 6 months before entering kindergarten; some of you have turned in the completed form. If you have not returned the report to the health office, please do so as soon as possible. Attached is a copy of the form (Report of Health Examination for School Entry). The form must be signed and dated by your child’s health care provider.
Your child may get the checkup from your regular source of medical care, such as a family doctor or pediatrician, or your child may be eligible for an examination at no charge. If you do not have your own health care provider and wish to know where your child may have a health examination, please call the district nurses at 925-462-5500, x4168 or the Child Health and Disability Prevention Program at 1-888-604-4636.
